The information requested in the form below is required to fulfill Thousand Island Pleasure Diving’s commitment to the U.S. Department of Homeland Security. We (TIPD) must submit an APIS (Advance Passenger Information System) passenger manifest 24 hours in advance of your dive. All divers wishing to dive with us in USA waters must submit this information. All information you provide to us will be held in the strictest confidence.
PLEASE ENSURE THAT YOUR INFORMATION IS CORRECT OTHERWISE YOU MAY CAUSE THE ENTIRE BOAT TO BE TURNED BACK AT CUSTOMS. ALL FIELDS ARE MANDATORY.
